I think that projects have about four dimensions, for this purpose: element, e.g. external envelope; location, e.g. first floor, block A (and recognise the hierarchy of locations); material, e.g. aluminium; and system, e.g. curtain wall. So the database could be queried to fetch all outstanding issues to do with, say, curtain walls, etc. I also want my software to relate any entry to a milestone (recognising the hierarchy of milestones) and in some cases a deliverable or set of deliverables (drawings, etc.), to give another method of collecting information to assist decision making.
